What the document says

Michigan MDHHS Bulletin MMP 26-47, issued October 30, 2026, and effective December 1, 2026, announces a rate increase for specified Maternal Infant Health Program (MIHP) services and revised documentation requirements for reimbursable care coordination activities. The bulletin distributes to Medicaid Health Plans, practitioners, hospitals, FQHCs, Rural Health Clinics, Tribal Health Centers, Local Health Departments, MIHP providers, and Highly Integrated Dual Eligible Special Needs Plans. Programs affected include Medicaid, Healthy Michigan Plan, MI Coordination Health, MIChild, and the Maternity Outpatient Medical Services Program. On the rate side, MDHHS will increase reimbursement for MIHP maternal or infant intake or initial assessment visits and care coordination services billed under HCPCS codes H1000, H2000, T1023, and T2022. The enhanced rates are reflected on the December 1, 2026 MIHP fee schedule. MDHHS states the increase leverages additional federal matching funds available to the state. On the documentation side, the minimum cumulative time required to bill care coordination services increases from 30 minutes to 60 minutes per beneficiary or infant-caregiver dyad per calendar month. Reimbursement for care coordination is conditioned on the prior establishment of an initial Plan of Care (POC), and all documented care coordination activities must reflect goals or interventions identified in that POC. Documentation must be completed by either the Licensed Social Worker (LSW) or Registered Nurse (RN) responsible for completing or delegating the activities. Each record must include a start time, end time, and detailed description of completed activities. Additional documentation standards are contained in the MIHP Operations Guide on the MIHP website.

What this may mean

Applies to all MIHP-certified agencies and MIHP home visitors billing HCPCS codes H1000, H2000, T1023, and T2022 under Michigan Medicaid and related programs effective December 1, 2026. The 60-minute cumulative threshold and POC-linkage requirement for care coordination put billing practices and LSW/RN documentation workflows at issue for any claim submitted on or after that date.
