What the document says

Michigan Department of Health and Human Services (MDHHS) issued Bulletin MMP 26-48 on October 30, 2026, effective December 1, 2026, to implement Section 5121 of the Consolidated Appropriations Act of 2023 (CAA). The bulletin applies to Medicaid, CHIP, and the Healthy Michigan Plan and establishes Medicaid coverage and reimbursement for pre-release screening, diagnostic, and Targeted Case Management (TCM) services for eligible justice-involved individuals. Eligible individuals are those who (1) were determined eligible for Medicaid or CHIP before or during incarceration, (2) are within 30 days of a scheduled release from an eligible correctional or juvenile justice facility, (3) are held post-adjudication, and (4) are either under 21 years of age or former foster care individuals up to age 26. The bulletin establishes two benefit plan structures. The REENTRY-MA benefit plan covers Medicaid-reimbursable screening and diagnostic services delivered in the 30 days prior to release. The TCM-INC benefit plan covers TCM services in the post-release period, with Michigan covering up to one year of post-release TCM. Pre-release services are reimbursable only when the eligible individual is enrolled in REENTRY-MA; post-release TCM is reimbursable only when the individual is enrolled in TCM-INC. Eligible facilities must confirm benefit plan assignment prior to release. Screening and diagnostic services for individuals under 21 must meet Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) standards, including adherence to the American Academy of Pediatrics periodicity schedule, and encompass comprehensive physical examination, vision, hearing, lab testing (including lead toxicity), dental screening, behavioral health screenings, and immunization assessment. For former foster care individuals up to age 26, screenings must be medically necessary to identify physical or behavioral health conditions. Diagnostic services must be rendered when a screening indicates the need for further evaluation. Previously administered screenings conducted at intake or during incarceration within a clinically appropriate time period (e.g., the last 12 months) may satisfy the pre-release requirement. TCM services, as specified under 42 CFR Section 440.169, consist of four core components: comprehensive assessment and periodic reassessment; development and maintenance of a person-centered care plan (to be completed within 14 days of initiating TCM); referrals and related activities connecting individuals to physical, behavioral health, dental, and social support services; and monitoring and follow-up activities, with at least one annual monitoring visit required. A warm handoff process is required when the pre-release case manager differs from the post-release case manager, with handoff meetings to occur prior to release when feasible. All data sharing must comply with HIPAA and the Michigan Mental Health Code. Provider enrollment requires both Type 1 (Individual) and Type 2 (Organization) provider entities to enroll in CHAMPS and obtain an NPI. Eligible Type 2 Organization providers include CCBHCs, correctional and juvenile justice facilities (pre-release only), CMHSPs, FQHCs, RHCs, Tribal Health Centers, and Tribal FQHCs. Reimbursement rates will be set by a CAA Reentry Services Fee Schedule published on the MDHHS Billing and Reimbursement webpage. Eligible facilities are required under CMS SHO #24-006 to confirm CAA compliance status and must submit a detailed justification if participation is not feasible.
