What the document says

The Illinois Department of Healthcare and Family Services (HFS) issued this provider notice on September 23, 2026, directed to Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IIDs) and Medically Complex for the Developmentally Disabled (MC/DD) facilities. HFS is conducting a program evaluation of the ICF/IID and MC/DD programs and, as part of that evaluation, is requiring all ICF/IID and MC/DD facility providers to submit individual-level acuity data directly to HFS. The acuity data requested covers all individuals for whom the ICF/IID or MC/DD facility provider is billing the State of Illinois. Specifically, HFS is requesting Inventory for Client and Agency Planning (ICAP) data and Health Risk Screening Tool (HRST) data. HFS acknowledges that ICF/IID and MC/DD facility providers are not currently required to complete the HRST on facility residents, and therefore recognizes that not all providers will be able to submit HRST data. HFS has established a SharePoint site called LTC Exchange to receive the submitted acuity data. Providers must first register by emailing user information, including user name, facility name, facility Medicaid ID, and a user-specific email address, to HFS.LTC.ICFIID@illinois.gov with the subject line "LTC Exchange User Information" by Wednesday, October 7, 2026. HFS will then associate each user with the relevant provider ID and send login instructions. Providers must upload the requested acuity data for all applicable facilities to the LTC Exchange site by Friday, October 23, 2026. HFS scheduled a webinar for Thursday, October 1, 2026, from 11:00 a.m. to 12:00 p.m., to walk providers through the upload process. A data submission template was attached to the notice and is also available on the LTC Exchange site.

What this may mean

Applies to all Illinois ICF/IID and MC/DD facility providers billing the State under the Illinois Medicaid program. Puts individual-level ICAP and HRST acuity data submission at issue, with a user registration deadline of October 7, 2026, and a data upload deadline of October 23, 2026. Providers without HRST data on file are not exempted from registration but are not penalized for inability to submit that specific data element.
