What the document says

A federal grand jury in the U.S. District Court for the District of Columbia returned a 10-count indictment, unsealed November 5, 2026, charging three individuals affiliated with Quality Healthcare Services, LLC, a Washington D.C.-based provider of Mental Health Rehabilitation Services (MHRS) to youth and adolescent Medicaid beneficiaries. The indictment charges Mwasi Songambele (controlling executive), Maya Cheyo (Chief Operating Officer and Human Resources Director), and Vanessa Kuh (community support worker) with conspiracy to commit health care fraud, substantive health care fraud counts, and, as to Songambele, conspiracy to commit money laundering. The indictment alleges that from January 2022 through February 2025, Quality Healthcare Services billed D.C. Medicaid approximately $14 million for MHRS. The alleged scheme involved two categories of false claims: billing for MHRS encounters that never occurred, and inflating the duration of telehealth encounters that did occur. Songambele allegedly directed employees to bill four units (approximately one hour) for all MHRS telehealth encounters regardless of actual call length, even when calls lasted only a few minutes. To avoid detection by the D.C. Department of Behavioral Health, the indictment alleges that management instructed co-conspirators to record fabricated session lengths between 53 and 67 minutes to make telehealth encounters appear more realistic. Songambele additionally allegedly directed employees to exhaust the full 600 units authorized per Medicaid consumer per 180-day period without regard to medical necessity or whether services were actually performed. The indictment seeks forfeiture of multiple luxury vehicles, including a 2024 Mercedes-Benz G550W4, a 2023 Mercedes-Benz G550W4, a 2022 Mercedes-Benz 2500 Cargo Van, a 2020 Mercedes-Benz C300W4, and a 2023 Rover Defender, as well as a residence on the 7000 block of Tranquility Road in Laurel, Maryland, and a Panthere De Cartier Watch. The investigation was conducted by the FBI Washington Field Office, the D.C. Office of the Inspector General's Medicaid Fraud Control Unit, and HHS-OIG.
