Summary

Michigan MDHHS issued Medicaid bulletin MMP 26-44 on October 30, 2026, establishing Fee-for-Service coverage and reimbursement for Licensed Midwife (LM) services under the Michigan Medicaid program effective January 1, 2027, pursuant to Public Act 22 of 2025, Section 1982. The bulletin covers antepartum, labor, delivery, and postpartum care for low-risk pregnancies in home and freestanding birth center settings, with LM services carved out of Medicaid Health Plans and reimbursed directly through FFS at physician professional service rates. LMs must enroll in CHAMPS with a Type 1 NPI, and FQHCs, RHCs, Tribal Health Centers, and Tribal FQHCs billing LM services face distinct institutional and professional claim requirements depending on care setting.

What this may mean

Beginning January 1, 2027, Michigan Medicaid FFS will reimburse Licensed Midwives directly for covered maternity services, carving those services out of Medicaid Health Plan capitation and requiring LMs to enroll in CHAMPS with individual NPIs. FQHCs, RHCs, and Tribal Health Centers providing LM services must follow separate institutional billing requirements for in-clinic services and professional claim requirements for out-of-clinic services. Freestanding Birth Center coverage remains contingent on state licensure and subsequent Medicaid enrollment of FBCs.
