Attention Care Coordinators: Atrezzo Coordinated Service Plan (CSP) Replaced by Emailed Supplemental Individual Support Plan (S-ISP) Questionnaire
What the document says
The Maine Department of Health and Human Services Office of MaineCare Services issued this provider bulletin on October 26, 2026, announcing that effective November 18, 2026, the Coordinated Service Plan (CSP) Questionnaire in the Atrezzo care management platform will be eliminated and replaced by a new Supplemental Individual Support Plan (S-ISP) Questionnaire delivered via email. Unlike the CSP, which was required for all children receiving Care Coordination, the S-ISP Questionnaire applies only to children covered by the Children's Behavioral Health Services (CBHS) DOJ Settlement Agreement. The questionnaire takes approximately 10 to 15 minutes to complete. Under the new process, a Care Coordinator receives an email with a link to the S-ISP Questionnaire when a Single Assessment is completed for a covered child. The Care Coordinator must complete the initial questionnaire within 30 days of the first meeting with the child and family to discuss Single Assessment results. A follow-up S-ISP Questionnaire is required six months after the initial questionnaire, with CBHS sending a reminder email approximately two weeks before the deadline. Care Coordinators are directed to continue using the existing CSP process in Atrezzo through November 17, 2026. No action is required for existing CSPs already in the system. For covered children who completed a Single Assessment before November 18, 2026, and do not have a CSP Questionnaire in progress, CBHS will send Care Coordinators an email link to complete the new S-ISP Questionnaire. An introductory training is scheduled for November 13, 2026.
What this may mean
Applies to Care Coordinators serving children covered under the CBHS DOJ Settlement Agreement in Maine MaineCare. Puts a 30-day completion deadline and a six-month follow-up obligation in place for S-ISP Questionnaires beginning November 18, 2026. The CSP Questionnaire requirement in Atrezzo is eliminated on that same date for all children receiving Care Coordination.
