Summary

The July 2, 2026 MLN Connects newsletter announces two major CMS proposed rules. The CY 2027 OPPS and ASC proposed rule includes payment rate updates, a proposal to reduce payments for 340B-acquired drugs based on a new Medicare OPPS Drugs Acquisition Cost Survey, elimination of the inpatient-only list, and new prior authorization requirements for additional botulinum toxin injection codes. The CY 2027 Home Health PPS proposed rule covers payment updates, expansion of the DMEPOS competitive bidding program, DME benefit expansion for infusion pumps and drugs, and changes to identical replacement item requirements. CMS also launched the Medicare GLP-1 Bridge effective July 1, 2026, with a $50 beneficiary cost share and prior authorization requirements for prescribers.

What this may mean

Hospitals and ASCs face proposed reductions in 340B drug payment rates based on a new acquisition cost survey methodology, elimination of the inpatient-only list, and new prior authorization obligations for additional botulinum toxin codes under the OPPS proposed rule. Home health agencies face proposed payment adjustments and expanded DMEPOS competitive bidding requirements if the HH PPS proposed rule is finalized. An OIG compliance item flags that some SNFs failed to accurately report related party costs on Medicare Cost Reports, resulting in overstated costs to Medicare.
